Referral
You may also be referred by an adult that you know - this could be a parent/carer, social worker, youth offending team, a teacher etc.
When the MATRIX receive your referral, we will arrange an appointment to see you at a time that is best for you. The staff understand some of the issues that you face and are friendly and approachable.

Assessment
When you visit the Matrix Service we will need to ask you some questions to find out the best way to help you. This is called an assessment.
The worker will explain how we can help you, what they will do with the information they gather, confidentiality and your rights.
You don't have to talk about anything that you don't want to and you won't be judged on the information you give.
Remember this is a service for young people and it is also your chance to ask the worker any questions that you have. After the assessment the worker will bring your file to the team to find the best worker to meet your needs, for example, if you prefer a male or female worker. You will then be introduced to your own worker for ongoing support.
Care Plan
A care plan is where you set your goals of what you want to achieve. This may be a reduction in your substance use or you might want to stop altogether.
The care plan can help you plan what you want to do and who can help you.
Review
After your care plan, you and your worker will look at your situation on a regular basis.
Sometimes your circumstances may change or things might become more important than others. It is at this stage that you can change your care plan depending on what you feel is important to you. You may have also achieved some of your goals and want to work on other issues.
Access
The MATRIX can help you get in touch with other agencies that can also help you if there are other problems you have e.g. training/education or other activities. We can help you access activities and contact Connexions on your behalf.
We will only talk to other agencies if we have YOUR permission to do so.
